We are looking for a SW IPC Manager to provide management support within our collaborative team (including people from a range of organisations) to enable the Right Skills to be developed within our Region to support personalised care.
The South West Integrated Personalised Care team is looking for colleagues to work with teams across the 7 South West systems to deliver Personalised Care to people in their communities. The Long-Term Plan places Personalised Care as one of the 5 major practical changes that will make the NHS into a 21st Century service. The role will include working with NHS organisations and key partners including Local Authorities and the Voluntary and Community Sector, to implement the Comprehensive Model of Personalised Care.
