South West Event: Care Home Residents with Dementia: How can we do better together?

Care Home staff report that their greatest challenge is in getting the right and timely support for people with dementia when they are deteriorating, often leading to an admission which could have been avoidable. Getting the right diagnosis and support requires a collaborative approach, coordinating primary care, community health services, older people mental health teams, secondary care as well as the care staff. This multi-professional, cross sector approach to the day intends to bring together the collaborative partners to focus on dementia diagnosis and support, with earlier identification of deterioration, to positively impact on reducing urgent care needs, avoiding admissions, improving discharge processes and improved outcomes for the individuals.
